Report A Claim

Everyone buys insurance hoping that they will never have to use it, but it is bound to happen. We know it is never a happy situation when a claim needs to be filed but we want to make it as painless as possible. You can file a claim in a number of different ways. Whichever works best for you in your particular situation.

First you can call us at 1-800-725-3358, second you can fax us at
1-918-540-0635, third you can call the insurance company directly. Please see your policy or wallet card information for the phone number for your particular plan.

Or you can file it online with our new claims service. Simply click on the appropriate line of coverage on the left side of this page and follow the prompts. Fill in your information as fully as you can then hit submit at the bottom of the page. You must be registerd to file claims online.  We will contact you to confirm your claim and get to work on it for you. Remember:
